These can be accessed both in the service user’s home and in-patient care whether for hospital and medical appointment, to delivery of excellent care from a specialised and dedicated care team, in the home, in-patient care and specialised treatment and admission at local hospitals and hospices.

    • Offering a high-quality service to those who choose to use our specialised team in providing individualised and best practice care when entering end of life care to both the service user and their family.
  • Local GP surgery – daily nursing and medical issues (day and out of hours), local pharmacy
  • District Nurse Hub – 0300 365 1234
  • Thames Hospice – 01753 848 925 – this is a 24-hour service available.            
    • These services include Nurse specialists, Psychological care and counselling, night sits with service user, social care and respite, in-patient, palliative care, Chaplin, occupational therapist, physiotherapist, hospice at home, social Worker, telephone support nurse, Rapid response, Macmillan and Marie Curie nurses alongside the district nurses. The hospice also offers free holistic therapies for service users and their families a wide range of various complimentary and holistic therapies

Accessing Services

Sometimes accessing services can seem difficult but it is important to remember that these services are there to provide support and care and that if there is a barrier to overcome, that there is a way to do this. Some of these barriers may include:

  • Disabilities
  • Immigration Status

 

If you are experiencing difficulties with accessing services, it is very important to remember that there are always several options available.
